Orthopedic Nurse Navigator Program
Orthopedics at Upstate Community Hospital provides hip and knee joint replacement patients with support throughout their treatment. Our Orthopedic Nurse Navigator program assigns a Registered Nurse to you that will work closely with you, your family, and your surgeon to guide you through your experience from surgery decision to post-operative care.
Your Orthopedic Nurse Navigator will work directly with you to:
- Provide education before and following your surgery.
- Develop a plan of care for after your surgery and determine what you will need to help you recover from your surgery.
- Help communicate with and coordinate care between all members of your healthcare team.
- Ensure seamless coordination of services.
- Visit/contact you periodically throughout your hospital stay to answer your questions and ensure a positive experience.
- Follow up with you when you discharge home as you recover.
- Answer any questions that arise throughout the process.
The goal of the Orthopedic Nurse Navigator Program is to ensure high quality care to promote your recovery and return to your normal daily living. One of our Nurse Navigators will be in touch with you prior to your surgery to explain his/her role and what you can expect throughout your surgical care.
