No action is required for those keeping their current health insurance option, if individuals still qualify for that plan.

The deadline for state employees to make changes to their health insurance carrier is Dec. 31.

To make a change, submit a signed health insurance transaction form PS-404 to the Upstate HR-Benefits Office, second floor Jacobsen Hall, (fax: 315-464-4390) or the Community HR-Office, first floor (fax: 315-492-5275).

Selecting a health insurance plan is an important personal decision. Consider family lifestyle, health, budget and benefit preferences. Think about what health care might be required during the next year. The changes for 2022 will be effective Jan. 6, 2022. If changing to a new option, most deduction changes will be effective later in January 2022 and will include retroactive adjustments in a future paycheck. Input sought for Covid vaccine survey with pediatric focus by Jan. 10

The pediatric Covid vaccine is safe and effective. However, vaccination rates for kids under 18 remain low. A team of vaccination and public health experts at Upstate developed a brief, five-minute survey to better understand preferences for vaccinating children against Covid.

The anonymous survey is open to everyone at Upstate and is available here: https://redcap.upstate.edu/surveys/?s=LE3PF7A87EEL8RN3. The information will help the team better understand barriers to vaccine uptake and improve access.

The survey link is also available on computer desktops throughout campus as an icon named, “Children COVID Vaccines.” The survey closes Jan. 10. Questions? Email Alyssa Indelicato at IndelicA@Upstate.edu.
