Connect Care

Connect Care provides high-quality, compassionate care to patients by providing telemedicine, acute same day and follow up appointments to patients ages 16 years and older. Individuals are of varied culture And socio-economic backgrounds. In addition to providing exceptional care, Connect care provides intermediate care between the Emergency Room and Hospital Discharge’s and the patient’s Primary Care Provider. Connect Care offers nursing assessment, patient education and teaching, coordination and continuum of patient care and referrals to community agencies and services.

Distinct Services Offered:

Connect Care - to facilitate hospital throughput and reduce non-emergency ED burden. This includes incorporating services and collaborating with departments who use inpatient and ED for non-acute needs as a way to facilitate workups. Operates during business hours to facilitate testing.

Services Provided

  • Intermediate Care between the Emergency Room and Hospital Discharge patients and Primary Care.
  • Patients who are discharged from the Emergency Room or from a Hospital stay that do not have a Primary Care Provider, seeking medical care post discharge.
  • Contacting and offering care to patients who are previously documented as leaving the Emergency Department, on their own accord, without completing a medical screening exam. Patients that are determined, based on nursing triage assessment, to have a non-emergency medical condition that can be treated within the scope of Connect Care services.
  • Individuals who left the Emergency Department prior to a medical screening exam will be subsequently identified and treated without discrimination with respect to any protected class or payment or insurance status.

Connect Care services include:

  • Infusion/ IV treatment
  • Medication administration
  • Assist specialty teams in Lumbar puncture
  • Paracentesis
  • Thoracentesis
  • Insertion of vascular access devices
  • Insertion of Implanted loop recorder.

Evaluation and non-emergency treatment of the following systems:

Cardiovascular, Ear/Nose/Throat, Endocrine, Eye, Gastrointestinal, Genitourinary, Hematologic/Lymphatic, Integumentary, Musculoskeletal, Neurological, Psychiatric, Rheumatology, Nephrology, Pulmonary, Dermatology, Internal Medicine and Geriatric are provided in-person or via telemedicine

In addition to providing direct patient care, Connect Care performs other important aspects of care such as nursing assessment, patient education and teaching, coordination/ continuum of patient care, referral to community agencies/services. Ancillary services we provide include Case Management, Care Management, and Social Work.

Services Connect Care cannot provide:

  • Difficulty Breathing/ New Shortness of Breath
  • Ob/Gyn & Active Labor
  • Dislocation or Fractures
  • Irretractable Vomiting/Diarrhea
  • Sepsis
  • Chest Pain
  • Respiratory Distress
  • Anaphylaxis
  • Primary Psychiatric Diagnoses/ Suicidal Ideation
  • Active Substance Abuse
  • Acute Concussion/Loss of Consciousness
  • Altered Mental Status
  • Any health emergency requiring a higher level of care
