Mission Statement

The Department of Medicine is committed to engaging medical students, residents, nursing, and our faculty in the quality improvement process. We wish to create a culture of safety in which each individual has a voice in recognizing and preventing adverse events.

This culture is based on transparency, self-reflection, and open communication. We encourage you to report any adverse events or near misses that you encounter using our reporting tool below. Please include relevant information such as patient name, medical record number, and date of incident. All submissions will be investigated and patient care issues will be discussed in our monthly quality improvement conferences.
