About the Division of Hospital Medicine
Hospitalists are healthcare providers who practice hospital medicine and deliver comprehensive medical care to acutely ill hospitalized patients. They have the skills to navigate one of the most complicated organizational structures — hospitals — and utilize all resources available to form high-quality and high-value healthcare plans for each patient.
Hospitalists work closely with hospital departments and healthcare providers to form the best evidence-based, patient and family-centered healthcare plans. In addition, they coordinate patient discharge processes to ensure safe and effective transitions.
The division is comprised of a team of healthcare providers and practice administrators. Healthcare providers include physicians and advanced practitioners at both Upstate and Community Hospitals.
