New Patients
Welcome to the Hematology/Oncology new patient page. If you are experiencing a medical emergency, please call 911 or go to the nearest emergency room.
Appointments are primarily made by physician request for consultation. Self referrals are accepted with proper supporting clinical records or for patients who are interested in a second opinion.
Making an Appointment
Appointments will not be scheduled until we have received the following information:
- Applicable records, including labs
- X-rays
- Patient demographic information, including a copy of the insurance card Patients should bring with them to the first appointment:
- A list of all medications, including over the counter medications
- A form of picture ID, eg. a drivers license
- Their current health insurance card(s) and any drug insurance card
- The completed history form that was sent in the mail
New patient consults should be prepared to spend a minimum of 1.5 hours in the office for an office visit which includes preparation, physician time, and discharge time. Subsequent visit times will depend on the patients diagnosis and plan of care and this will be discussed with you at the initial visit.
