Outpatient Continuity Program
The continuity program is a major aspect of our primary care training and is part of current efforts to enlarge and broaden the primary care experiences for the residents.
One day each week residents attend their own outpatient primary care continuity clinic. By seeing patients on the same day of the week over three years, residents are able to follow patients through different stages of growth and development and form a long-lasting bond. This is one of the most rewarding aspects of residency.
Beginning at the start of the PL-1 year, each resident becomes the primary physician for a panel of families to follow throughout the residency in order to gain experience in the continuity of care to children. These families are followed for three years as part of our Pediatric Outpatient Continuity Program. Each resident belongs to a group practice team that includes pediatric residents, nurses, nurse practitioners and 2 to 3 faculty members.
Family panels grow at the resident's discretion with guidance from their team attendings. They represent the whole spectrum of pediatric practice, including well babies, children with chronic illnesses, subspecialty diseases, and behavioral and psychosocial problems. Children who are part of this program and who require inpatient care are provided with coverage by their group practice resident and a general pediatric faculty member. Twenty-four hour telephone coverage is also provided, with a program for training residents in telephone triage. A record is kept of individual house staff experiences with specific outpatient diagnoses and an effort is made to maintain a broad range of experiences for the resident. Residents are freed from inpatient responsibilities during their weekly scheduled group practice sessions. The program continues to enlarge so that currently over 9,000 children are enrolled in the program. Our program is relatively unique in having residents truly own patients as PCP—practically for family's recognition and in documentation/communication in the EMR.
