Core Training Sites

Upstate Golisano Children's Hospital: Level 1 Pediatric Trauma Center, Regional Pediatric Emergency Department, Clark Burn Center, Upstate NY Poison Control Center.

SUNY Upstate Medical University is the primary site for the fellowship. This is a tertiary care, referral hospital servicing 22 counties with a full complement of pediatric medical and surgical subspecialists. Fellows work with multiple attendings each rotation and have access to numerous subspecialists, ancillary staff, and support personnel. The fellowship director, associate director and program coordinator are all located at this site. Rotations at this site include 3 of the 4 Inpatient Medicine rotations each year, and most elective rotations and scholarly activities. At Upstate, fellows learn the management of straightforward and medically complex pediatric patients and the co-management of surgical and subspecialty patients. They also learn to navigate the health care system and how best to utilize ancillary services like pediatric inpatient pharmacists, case management, social work and child life. Furthermore, they hone their teaching and research skills through a variety of scholarly activities including acting as teaching attending, serving as a Practice of Medicine 1 course instructor, preparing educational modules for the residents and medical students at both SUNY Upstate campuses, participating in quality improvement projects, and presenting at state, national or international conferences. The Pediatric Inpatient Medicine Service at Upstate admits over 3000 patients per year.

Crouse (newborn nursery and NICU): Regional Perinatal Center

Crouse Hospital is the Regional Perinatal Center for Central New York and the site of the fellowship's NICU/newborn nursery core rotation. Fellows may also choose to spend additional time at Crouse during an elective rotation. Fellows work under the supervision of pediatric attendings in the Kienzle Family Maternity Center where approximately 4000 newborns are delivered per year. They also work under the supervision of neonatology attendings at the Walter R.G. Baker NICU which houses over 50 isolettes and treats over 900 infants per year. Clinical responsibilities range from routine newborn care and anticipatory guidance to stabilization and management of critically ill neonates. Crouse is located next to Upstate and connected via pedestrian bridge.

United Health Services Hospitals (community site):

UHS is the site of the Community Hospital Medicine rotation and a site which offers opportunities for scholarly activity. Fellows spend 1 month at UHS in their second year under the supervision of board-certified pediatric, neonatology and family practice attendings. There are a limited number of board-certified pediatric subspecialty providers and ancillary staff available on-site. However, SUNY Upstate PHM faculty including the program director and associate director are available for phone consultation when fellows need additional guidance. At UHS, fellows learn to differentiate between patients who can be safely managed in a community hospital setting and those requiring transfer for subspecialty or higher levels of care. Clinical responsibilities include managing admitted pediatric patients, providing consults in the Emergency Department, stabilizing pediatric patients for transfer, and performing procedures as needed.

In addition to clinical experiences, fellows develop and provide learning modules on general pediatric topics for the UHS family medicine residents. They also assist in preparing the residents to educate the medical students on these topics. Furthermore, fellows teach pediatric physical exam skills to SUNY Upstate medical students on clerkship rotations at the Binghamton clinical campus and provide informal learning opportunities at bedside.
