Incidents or concerns of discrimination, harassment, or retaliation that you believe to be based on actual or perceived race, color, national origin, religion, age, disability, sex, gender, pregnancy, gender identity, gender expression, sexual orientation, transgender status, predisposing genetic characteristics, marital status, familial status, veteran status, military status, domestic violence victim status, criminal conviction record, or other legally protected category.

Sexual harassment, sexual assault, dating/domestic violence, or stalking:

Incidents or concerns of sexual harassment, sexual assault, dating/domestic violence, or stalking.
