Patient Population

The Wound Care Center provides services for adult outpatients (18 and older) referred by a Physician/Primary Care Provider for evaluation and treatment of problem wounds. (Patients younger then 18 years old will be considered on an individual basis according to diagnosis.)

Clinical Interventions

• Physician consultation and ongoing care
• Treatment plan development
• Patient education
• Referrals for laboratory studies, radiologic studies, vascular studies, social work, case management, nutrition, discharge planning and instruction
• Advanced treatments
