Event Approval & Room Reservations
Complete Step 1 and Step 2 at the same time. If there are issues you will receive an email from the Office of Campus Activities. Upon receipt of the room confirmation email your event is approved, room is secured, and you are able to move forward with your planning process.

Frequently Used Forms
Forms may be printed and turned into the Office of Campus Activities or the CAB Information Desk. While we hope to have a fully automated system in the future, hard copies are still required for all forms to be turned in with appropriate signatures. Forms are still available to be picked up at the CAB Information Desk or the Office of Campus Activities.
Fill out for all reimbursements or invoice payments.
Utilized for any changes in an organization's e-board or after elections.
