The Problem of Classification in Psychiatry: Instrumental Validity and Ethical Foundations

Ronald W. Pies, M.D., Professor of Psychiatry and Lecturer in Bioethics and Humanities, Upstate Medical University, and Clinical Professor of Psychiatry, Tufts University School of Medicine, Boston, MA and Robert W. Daly, M.D., Professor Emeritus of Psychiatry and of Bioethics and Humanities, SUNY Upstate Medical University, Syracuse, NY

This talk will review the general concept of "diagnosis" in clinical medicine; explore ideal features of diagnosis (such as reliability and validity); discuss the philosophical underpinnings of psychiatric diagnosis; and present an ethics-based concept of diagnosis, founded on the construct of "instrumental validity." I will present some of the pros and cons of categorical, dimensional, and prototypical diagnostic schemes. Finally, I will argue that the ultimate goal of psychiatric diagnosis is not "carving Nature at its joints", but rather, the effective relief of human suffering and incapacity.
