Director

Beth Erwin , CPCS, serves as Director of Medical Staff Services and as such is responsible for the supervision and direction of all activities of the Medical Staff Office. She can be contacted at Room 204 CWB, or (315) 464-5733.

Assistant to the Director / Physician Credentialing

Michele Smelkoff is the Assistant to the Director of Medical Staff Services, and a Credentialing Coordinator. She can be contacted at Room 205 CWB, or (315) 464- 5653.

Health Professional Credentialing

Chris Lamanna is the Health Professional (HP) Credentialing Coordinator and also the Administrative Assistant to the Medical Alliance at University Hospital. She can be contacted at Room 206 CWB, or (315) 464-5654.

Credentialing Coordinator

Mike Praskey is a Credentialing Coordinator. He maintains and tracks physician mandatory education, and quality data.  He can be contacted at Room 208 CWB, or (315) 464-5651.

Resident Credentialing Coordinator

Geraldine Dawson is the Resident Credentialing Coordinator. She tracks all licensure andcertification for resident and physicians. She can be contacted at Room 210 CWB, or (315) 464-5658.

Mailboxes are located outside of Gerry's office, and outside the Resident's lounge, for Resident procedure cards.

Credentialing Coordinator

Elyse F. DeLawyer is a Credentialing Coordinator, and the Quick Preliminary Check contact. Elyse maintains the SMS CAIS Doctor Master for privileged practitioners.  She can be contacted at Room 211 CWB, or (315) 464-5655.
