Becoming a Transplant Candidate

The potential transplant candidate is

• referred by a nephrologist
• scheduled for full evaluation day
  (Guided thru process by a recipient)

1. education session
2. history & physical
3. social worker evaluation
4. financial coordinator evaluation
5. reviewed by surgeon
6. extensive lab testing (additional testing as needed)

• all findings reviewed by transplant committee
• (according to findings) placed on active waiting list
