How to File a Complaint
Patients may receive a customer satisfaction survey at home after discharge or after an outpatient appointment. We would appreciate your taking the time to let us know about your experience at University Hospital.
Filing a Complaint:
If you have a complaint about the quality of care provided, we encourage you first to bring the complaint to the attention of the hospital's management by contacting our Patient Relations Department.
If you or your representative feels that the Patients' Bill of Rights has been violated, we encourage you to let us know by contacting:
