How to File a Complaint

Patients may receive a customer satisfaction survey at home after discharge or after an outpatient appointment. We would appreciate your taking the time to let us know about your experience at University Hospital.

Filing a Complaint

If you have a complaint about the quality of care provided, we encourage you first to bring the complaint to the attention of the hospital's management by contacting our Patient Relations Department. If you or your representative feels that the Patients' Bill of Rights has been violated, we encourage you to let us know.

You can also call our University Hospital Patient Safety Hotline to report concerrns anonymously or for immediate help.

Grievances will be addressed within 30 days unless circumstances indicate otherwise. We are committed to continually improving our care and delivering MEDICINE AT ITS BEST.

Greivance Committee

If Patient Relations is not able to resolve your issues, you can request information from the Patient Relations office about the Grievance Committee review process and how this process may be used to help solve your concerns. Based on the nature of the concern, a review by the Grievance Committee may be in order. Based on the findings of the Grievance Committee, a written response will be sent containing the final determinations. If you are unhappy with this response you can contact:

If you are unhappy with how your complaint is being resolved by the hospital you also have the right to bring the complaint to the Joint Commission:
