Patients may receive a customer satisfaction survey at home after discharge or after an outpatient appointment. We would appreciate your taking the time to let us know about your experience at University Hospital.
At Upstate Medical University patients and family have the right to voice questions, concerns and/or any complaints about the care and services provided at University Hospital. All concerns will be addressed in a supportive and respectful way.
If you have a concern about the quality of care provided, we encourage you first to bring your concern to the attention of the hospital's management by contacting our Patient Relations Department.
Contact information can be found at the top of this page. If you or your representative feels that the Patients' Bill of Rights has been violated, please let us know.
Grievances will be addressed within 30 days unless circumstances indicate otherwise. We are committed to continually improving our care and delivering MEDICINE AT ITS BEST.
