Accessing the University Asthma Center
Your primary care physician may refer you to the Asthma Center. Or, you may contact us directly. You are welcome to phone Health Connections at 1-800-464-8668 for the following services:
- To arrange for an appointment or to learn more about our program
- If you are not ready for an appointment, you may wish to request our Express Information Library catalog which gives you free phone access to information on a range of topics, including asthma.
- Consult the Health Information Library, a service of the SUNY Upstate Medical University, to get free, in-depth printed information on asthma, or any other disease.
Preparing for Your First Visit
You will be scheduled for one or more visits with a lung physician. In addition, you will have three or more appointments with our certified asthma nurse educator. Be prepared to fill out a complete health history. If available, bring all information pertaining to your previous treatment and any chest x-rays you have had. And, by all means, ask questions and discuss any concerns you have with us. Write them down beforehand to help you remember.
Insurance Issues
Your insurance plan may require a referral from your primary care physician before you can access our services. If not, you may contact us directly.
