Your First Appointment

Your initial appointment will usually be scheduled following a referral from your primary care physician. Please bring with you all medical records, X-rays, CT scans, pathology slides, etc., current insurance information, picture identification and current list of all medications. Also, please bring money for parking as we cannot validate parking.

Please allow 1-3 hours for your visit.
