Becoming a Transplant Candidate

The potential transplant candidate is

• Referred by a nephrologist
• Scheduled for full evaluation day (guided thru process by a recipient)
  1. Education session
  2. History & physical
  3. Social worker evaluation
  4. Financial coordinator evaluation
  5. Reviewed by surgeon
  6. Extensive lab testing (additional testing as needed)
• All findings reviewed by transplant committee
• (According to findings) placed on active waiting list
