A 5K walk and run to benefit the American Heart Association's fight against Heart Disease and Stroke, America's #1 & #3 killers.

Upstate has been a proud sponsor of the Heart Walk for many years, and Team Upstate has been the top overall fundraising team for the past SIX years. Upstate was also awarded the Heart Failure Silver Award and the Stroke is Gold Plus Award for the last two years. Upstate's cardiovascular research program has received hundreds of thousands of dollars from the Heart Walk’s proceeds over the years.

How to Register for the Upstate Team & receive a long-sleeve Team Upstate shirt - View Shirt

1. Register as a walker ON-LINE -or- Pick up a paper registration form from any TEAM CAPTAIN.
2. Donate a minimum of $30 ($15 for students) to qualify for a long-sleeve Team Upstate shirt.
   • donate on-line by credit card or return your donation to your Team Captain (cash or check payable to American Heart Association)
• or return completed paper registration form and minimum $30 ($15 for students) to your Team Captain
  note: the American Heart Association has instituted a $25 minimum registration fee this year - but they are still allowing our students and children to register for $15.
  

You MUST give your shirt size to your team captain by March 6 - even if you register on-line!

Deadline to qualify for a Team Upstate shirt is MARCH 6 - you may register right up to the event, and also at Heart Association registration tent on the day of the event, but after 3/6 there will be a limited amount of shirts available on a first-come first-served basis.

Employees, students, friends and families qualify for a shirt - just register for the Upstate team!

Youth: XS (2-4), S (6-8), M (10-12), L (14-16)
Adult: S, M, L, XL, 2XL, 3XL, 4XL, 5XL

If you do not have a team captain in your area, call 464.4836 to join a team (Upstate Community General campus, call 492.5778).

Continue to Fundraise!

• Help Upstate maintain the title of top fundraising team - continue to fundraise right up to the event!
• Additional donations can be turned in at the American Heart Association tent on event day.
• Participants who raise $100 or more will also receive the 2012 Heart Walk t-shirt! Participants who raise $500 or more are members of the 500 Club and earn additional awards.
