  IRB Revised New Submission Deadline - December Meeting
  This deadline is for studies that have undergone primary scientific review and required full board review (as noted on the letter to the PI)
  
  Submit the following as requested:
  
  -A letter addressing issues raised by reviewers (if applicable)
  -One copy of the amended IRB application (parts A, B, and schematic diagram) and consent document(s) with all changes clearly marked or highlighted
  -One copy of the amended IRB application (parts A, B, and schematic diagram) and consent document(s)
  -25 copies (unmarked)of the IRB application (parts A, B, and schematic diagram) and consent document(s)
  
  Please note:
  -The IRB number must be written on the first page o each copy of the application (where indicated)
  -The PI must sign the amended application
  -The application must be stapled together (parts A, B, schematic diagram, and consent document) for each of the 24 copies

  This deadline is for studies that have not previously been submitted to the IRB.
  
  Please submit all requested items to ensure a timely review:
  -One original and two copies of the IRB application form (parts A and B) and the consent document
  -Two copies of the research protocol (if separate)
  -Two copies of the investigator's brochure or report of prior investigations (if using an investigational drug or device)
  -Two copies of the grant application (if applicable)
