CALGB 30610 (RTOG 0538):

Phase III Comparison of Thoracic Radiotherapy Regimens in Patients with Limited Small Cell Lung Cancer Also Receiving Cisplatin and Etoposide

Inclusion/Exclusion

Limited small cell lung cancer; no surgical resection, prior chemo, or prior RT; PS 0-2; staging CT chest/abdomen, bone or PET scan, & MRI or CT brain prior to study entry

Details: All study subjects will receive 4 cycles of the following chemotherapy:

  • Cisplatin 80 mg/m2 IV over 60 minutes on Day 1, every 21 days
  • Etoposide 100 mg/m2 IV over 60 minutes on Days 1, 2, & 3, every 21 days

Subjects will be randomized to one of these three radiation schedules:

  • Arm I—Radiotherapy every day, Monday–Friday, for a total of 3 weeks
    1.5 Gy twice daily, starting on Day 1 (total = 45 Gy)
  • Arm II—Radiotherapy every day, Monday–Friday, for a total of 7 weeks
    2.0 Gy once daily, starting on Day 1 (total = 70 Gy)
  • Arm III—Radiotherapy every day, Monday–Friday, for a total of 5 weeks
    1.8 Gy once daily, starting on Day 1 for 16 days of treatment;
    then 1.8 Gy twice daily for 9 days of treatment (total = 61.2 Gy)

Subjects may choose to participate in the optional quality-of-life and specimen submission components of this study.

A Randomized Phase III Comparison of Standard-Dose (60 Gy) Versus High-Dose (74 Gy) Conformal Radiotherapy with Concurrent and Consolidation Carboplatin/ Paclitaxel +/- Cetuximab in Patients with Stage IIIA/IIIB Non-Small Cell Lung Cancer

Inclusion/Exclusion

Newly diagnosed, unresectable Stage IIIA or IIIB non-small cell lung cancer; PS 0-1; patients with supraclavicular or contralateral hilar adenopathy are ineligible

Details: Subjects will first be stratified according to the form of radiation they will receive, their performance status, whether a baseline PET scan was performed, and the histology of their tumor. They will then be randomized to one of the following treatment arms:

  • Arm A—Radiotherapy once daily, Mon-Fri, beginning Day 1, for 6 weeks (total = 60 Gy)
    Concurrent carboplatin & paclitaxel on Days 1, 8, 15, 22, 29, & 36
  • Arm B—Radiotherapy once daily, Mon-Fri, beginning Day 1, for 7.5 weeks (total = 74 Gy)
    Concurrent carboplatin & paclitaxel on Days 1, 8, 15, 22, 29, 36, & 43
  • Arm C—Radiotherapy once daily, Mon-Fri, beginning Day 8, for 6 weeks (total = 60 Gy)
    Cetuximab loading dose on Week 1, Day 1
    Concurrent carboplatin, paclitaxel, & cetuximab on Days 8, 15, 22, 29, 36, & 43
  • Arm D—Radiotherapy once daily, Mon-Fri, beginning Day 8, for 7.5 weeks (total = 74 Gy)
    Cetuximab loading dose on Week 1, Day 1
    Concurrent carboplatin, paclitaxel, & cetuximab on Days 8, 15, 22, 29, 36, 43, & 50
