I couldn't have found a more rewarding program than the one that I have found here at Upstate Medical University

Applicants should download and complete the form below, submit the standard APPIC application form and a vita, and arrange to have sent to the Division a transcript of graduate studies and at least three letters of recommendation from faculty or supervisors. These may be sent in the applican'ts packet or sent individually from the programs.DEADLINE FOR SUBMISSION OF APPLICATIONS IS DECEMBER 1st. We are suggesting that intern applicants get their material in to us as early as possible so that the application can be carefully considered by the selection committee.

The standard "Verification of Internship Eligibility and Readiness" form and/or one of the letters should be from the Director of the Graduate Clinical Program. The applicant should feel free to include any special material or details of experience that might provide the Division with additional information.
