Special Events

Students admitted to one of the Summer Career Awareness Days were sent an e-mail confirming their acceptance and will need to submit to the Office of Admissions (see address below) the following paperwork at least 2 weeks prior to the date of the event.  Final information including directions and a campus map will be e-mailed to the participant one week prior to the event.  Please be sure to look out for it, including in the SPAM filter.

NOTES:
On the Certificate of Health form, only complete the highlighted questions.  Also, please add any additional health issues the participant has to the bottom of the page. If the student becomes sick or injured during the event, this form will be given to the health care practitioner.

Some events may be in places where patients are located which is why the HIPPA and Confidentiality information is included, though we're not planning any activities directly involving patients.

Emergency contact - please include the person who would be able to pick up the participant in case of an emergency.

Also, here is a list of regional colleges and things to do in Syracuse.  If you plan to arrive the day before the event, or stay after the program, there are many colleges about an hour or less drive from Syracuse that you can visit.  Please note, you will need to set up any visits yourself prior to arriving.  The Summer Career Awareness Day is for the participants only, so if a friend or family member is dropping you off, let them know there are fun things to do in the area.  Or you can do so after the event!
