Applicants must complete two forms:
  • On-line
    Application for: BS, BS-MS, MS, Post Masters
    or download an Application
  • RN First Assistant Application PDF document text
  • SUNY Application:
    required for BS, BS-MS students only.
    The general SUNY application will be sent directly to applicants requiring it.
 Graduate
  • Graduate Applicants are not required to complete the SUNY Application
