Undergraduate Transfer Applicants must complete two forms:

College of Health Profession Application

State University of New York Application 		Graduate Applicants:

Medical Technology, MS

Doctor of Physical Therapy:

  • No SUNY Application is required.
  • DPT Application Deadline: February 1

NOTE: High school seniors and first year college students applying forearly admission are not required to complete a SUNY application at this time.
