Check List

The following is the list of conditions that you must fulfill prior to matriculation...

• Health Forms are due 15 days prior to the start of classes
• A final, official transcript(s) from every college(s) ever attended
• Your final, offical transcript should indicate all coursework taken and the degree you have been granted.
• Completion of all premedical courses required for admission and all other courses with no grade lower than a "C"
• Completion of your academic program
• Read the technical standards and sign off that you have read them
