Appointments/Referrals
To schedule an appointment with one of our physicians, please call our office at (315) 464-4472. Our office is open 8:00 a.m. - 5:00 p.m., Monday through Friday. Some insurance plans require a referral from your primary physician prior to approving specialty care such as that provided by our group. Please ensure that you are eligible for care prior to scheduling an appointment. This policy applies to x-rays and any other procedures that require prior approval by the insurance provider. Some insurance plans allow you to choose the physician who will provide your specialty care, with or without your primary physician's approval. Check with your insurance company or primary care physician for more information.
What to Bring to Your First Appointment
- Previous x-rays and test results relating to your injury or illness
- Previous medical records from referring physician relating to your condition
- Your valid insurance card(s) and HMO/PPO referrals
- Referral letter (office notes) from your primary care physician
