Insurance Information
Our office accepts most major insurances including:
- Aetna
- Cigna
- Excellus
- Medicare
- Medicaid
- Total Care
- Pomco
- GHI
- Fidelis (Medicaid only)
As our participation with various insurers may change, please consult your Provider Directory to ensure that we are listed as current network providers.
We will bill your insurance for all services rendered. At the time of the appointment, we do require that you make the co-payment designated by your benefits plan. If you have an outstanding balance that has been determined by your insurance to be “patient’s responsibility” (i.e., deductibles, co-insurance, etc.) and you have not paid by mail, we will also collect that amount at the time of your office visit.

The office DOES accept Master Card and Visa for your convenience.
Workers Compensation and No Fault Insurance
If your visit is related to a work injury or an accident and is to be filed with Worker’s Compensation or No Fault carrier, please make sure to bring:
- Claim number
- Address and phone number of the insurance
- Date of the injury
- Name and phone number of the case manager handling your claim
