Internships

Job Internships occur in University Hospital departments other than Nursing - i.e., Dietary Nutrition, Hospital Administration, Radiology, Pharmacy, Social Work. These internships will be handled by the individual departments and all related forms should be sent directly to the department providing the intership.

Before you begin this process the department will need to verify with Rich Groth, Contract's Office, that an affiliation agreement between the student's school and SUNY Upstate Medical University exists. Once this is confirmed, the following forms and education will need to be completed.

Required Forms

• Assumption of Risk Release Form text
• Upstate Medical University Confidentiality Agreement text
• Upstate Medical University Detailed Health Statement text (will be at SUNY Upstate greater than 3 days)
• Upstate Medical University Short Term Health Statement text (will be at SUNY Upstate 3 days or less)
• Signed HIPAA Privacy Rule Education Completion Certificate text *after completing below HIPAA self study PowerPoint presentation.
• Non-Employee ID Request Form- text Required for Admittance

Requirements

• The initiating department will be responsible to maintain documentation of required forms and forward copy to appropriate department.
• Guidance Counselor or Partners in Business, or a sponsoring department must assist with placement and ensure forms are submitted and HIPAA self study is complete
• Student must be minimum of 15 years old *(Pediatrics In-Patient Child Life Specialist - learner must be at least 18 years old).
