Annual Performance Evaluation Process

Effective September 1, 2006

Performance Program:

  • State Form
  • Job Description
  • Annual Core Competencies (Staff, Supervisor, Director, Nursing Director, etc.)

Performance Evaluation:

  • State form with narrative evaluation based on job description.
  • Completed Annual Core Competencies (Staff, Supervisor, Director, Nursing Director, etc.)

Competency Assessment Process

Effective September 1, 2006

The following competencies are completed upon initial hire PP/PE cycle or upon a change in title. Then they should be completed every three years thereafter. Once completed they are sent to HR for logging & filed in official personnel folder.

  • Job Specific Competency Assessment
  • Unit Specific Competency Assessment
  • Population Served Competency Assessment
