When May I Use MyChart?
The program was implemented in January 2012, beginning with Upstate's Family Medicine practice, and will expand incrementally to cover all Upstate ambulatory patients by the end of the year. Ask about MyChart during your next clinic visit.
How Do I Use MyChart?
Patients who wish to participate will be issued a MyChart activation code during their clinic visit. This code will enable you to login and create your own user ID and password.
