The Upstate Transfer Center is a dedicated phone service staffed 24/7 by experienced registered nurses to facilitate all medical or surgical patient transfers to University Hospital and the Upstate Golisano Children's Hospital.

- Reason for transfer request

- Summarize the care provided so far
- Your recommendations for the patient's care
- Existing infection control concerns
- Special needs (interpreter, bariatric equipment)
