Prescription Renewals
Prescription renewals for established patients may be requested by calling the outpatient office during regular office hours.

Before the prescription request can be reviewed, patients will be required to clearly state:
- Name of the medication
- Strength
- Dose
- Pharmacy name
- Pharmacy phone number
Patients may be required to schedule a follow-up appointment for a periodic examination between prescription renewals.
