Incoming Resident/Fellow Required Health Forms
The following forms must be completed and returned to Employee/Student Health by June 1st. Please refer to the Health Requirements for Residency memorandum for specific instructions and requirements.
- Medical History/Physical Exam
Incoming residents must provide a physical exam by a licensed health care provider done within 6 months. - Allergy History
- OSHA Medical Questionnaire (Complete questions 1-9)
- Resident Immunization Record
- Resident Release of Information Authorization
All of these forms should be mailed or faxed to Employee/Student Health prior to arrival in Syracuse for the health office staff to review. If you were a former employee, volunteer or student, please contact Employee Student Health prior to completing the forms.
If you are in the Syracuse area prior to orientation, you can contact Employee/Student Health at (315) 464-4260 to arrange a tuberculin skin test placement and respirator fit-testing. This will save time at orientation.
