Training Verification/Duplicate Certificates

Instructions for requesting Training Verification/Duplicate Certificate:

  1. Complete the Training Verification/Duplicate Certificate Request Form.
  2. Either mail payment to the GME office or call the Bursar's office at 315-464-5148 to make payment. Fees must be received prior to the verification being completed. Only Syracuse affiliated hospitals are exempt from payment.
  3. Download a copy of Authorization and Release Form PDF document, sign and submit it online with the verification request or send it to the GME office via fax or mail.
  4. Allow from 4-6 weeks for verifications and up to 3 months for duplicate certificates.
