Clinical Services Provided
- Transition children to inpatient rehabilitation (acute/subacute) facilities when needed
Support Services Provided
- Assess and verify health insurance coverage. Advocate on patients behalf as needed.
- Coordinate appropriate resource utilization and communication between team members for discharge.
- Ensure that child and family have safe transportation to home and appropriate car seat when indicated.
- Make appropriate home care, home infusion and durable medical equipment referrals for safe transition home.
- Meet and assess 100% of admissions within 24-72 hours. Explain role of case manager and assess needs for discharge.
- Participate in multi-disciplinary rounds and make referrals to services as needed.
- Pharmacy and infant formula liaison. Ensure that any difficult to find medications/formulas are in stock and available before child is discharged.
- Refer to early Intervention, outpatient therapies, WIC, Syracuse Fire Dept., developmental service org., or other agencies as needed for safe discharge environment.
