Going Home

Discharge planning

Preparing for your child's discharge from the hospital requires careful planning. The Case Manager, PSL and Social Worker will help you obtain any additional services or equipment that you may need. Your physician will discuss the plans for discharge ahead of time.

Please plan on leaving the day of discharge before 11 AM.

Safety Seat Required

A car seat is required by law for children under 40 pounds. If you do not have one please let your nurse or case manager know and they will help you get one.
