Certificate of Health

Part 405 recently amended New York State Health Department regulations. This regulation requires the SUNY Upstate Medical University to maintain health records for all persons affiliated with the University.

There are no exceptions, all students must complete this form: Certificate of Health

The New York State Health Department will not permit SUNY Upstate to allow you to participate in any training activity without the required documentation. As test results must be verified before you begin, waiting until you arrive to take any of the required tests will delay your beginning of the rotation.
