Apply for a valid passport.

You may apply at a local County Clerk's Office, such as the one in Onondaga County, located at 401 Montgomery St, Syracuse, NY 13202. Phone: 315-435-2235

Apply for a visa to the country where you will be studying if necessary. Review the visa requirements to foreign countries at http://www.travel.state.gov/visa/visa_1750.html
Note: For the Prague Selective: USA citizens do not need a visa to enter the Czech Republic. Citizens from other countries should obtain information regarding possible visa requirements. A list of countries from which a visa is required for entry into the Czech Republic is at www.czechembassy.org

Student Health at SUNY Upstate does not provide immunization services. However, here is a list of several physicians in the area who can determine and administer the immunizations you will need for your trip abroad:

Please remember to contact the physician in advance to provide information about the location of your travel and to make an appointment. The doctor’s require cash payment as the immunizations fees are not covered by health insurance.

International Travel Insurance - MEDEX - available through SUNY http://www.medexassist.com/Groups/WhoWeServe/Scholastic.aspx

MEDEX offers global travel protection and 24-hour emergency medical and security assistance as well as medical, social and political evacuation and repatriation programs. Cost: $6.00 per month for the 2007-2008 academic year. Coverage may be purchased at the Bursar's Office.

Q. How do I receive academic credit for my experience?
  • Request an acceptance letter on letterhead from the institution or organization where you are completing the elective. The dates of attendance should be noted on the letter.
  • Once you have received the acceptance letter, complete a Medicine Add/Drop Form and a Unique Elective Form.
    • Both forms should be signed by the relevant clinical department chair (e.g. Medicine, Peds, etc.) and Dr. Lynn Cleary, Sr. Associate Dean for Education (Curriculum Office)
    • Please work with your faculty sponsor to coomplete the unique elective form.
    • Dr. Cleary will use the unique elective form to determine the academic credit that should be associated with the unique elective.
  • Submit the completed, signed Add/Drop and Unique Elective Forms and acceptance letter to the Registrar's Office to be processed (Room 204, CAB)

    Note: International students are eligible to apply for research positions, but not work study related summer opportunities.
