FPMHNP—How to Apply
We are admitting students to the Family Psychiatric Mental Health Nurse Practitioner (FPMHNP) track in our Master's Program or transferring those students who are in other tracks and wish to complete the FPMHNP track. If you wish to apply for this track in the master's or post-master's program, you may do so through the Admissions Office.
The application deadline for the Fall semester is January 15th. Deadline for completion of Upstate Supplemental Application and submission of supporting documents is February 15th. We no longer accept applications for Spring semester to the graduate program

- Official Transcripts from all colleges previously attended.
- Three Letters of Recommendation from faculty, employers and/or other people in a position to judge your potential for graduate study. One letter must be from someone able to evaluate your clinical work experience. These forms are in the application booklet.
- TEST SCORES: from BS level Excelsior College Exams (ECE) used to fulfill admissions requirements. (This applies only to applicants who have a degree in a field other than nursing.)
- RN license, CPR certification: Copies of your current RN license and current CPR certification.
- $65 Application Fee Make payable to: SUNY Upstate Medical University. If you are using the on-line application process, please also include the signature page at the end of this process with your application fee. (fee is waived for current Upstate students)
If you are a matriculated student who would like to transfer to this track, please contact Margaret Wells for the master's program or Barbara Black for the post-master's program. Contact information is given below.
