You will receive from your DEPARTMENT REPRESENTATIVE:

• Research Pledge Card text
  (Last year's total pledge is pre-printed on pledge card.)
• Research Agency List
• Green Foundation Card
• Return envelope

• Choose one method of payment: cash, check or payroll deduction (begins January 2008 for 26 pay periods).

PLEASE REMEMBER TO SIGN YOUR PLEDGE CARD!

• If you designate to a United Way or Upstate Foundation agency, please make check payable to the United Way
• if you designate to a Community Health Charities agency, please make check payable to Community Health Charities.
  
• Make your designation choice(s) - see Research agency list(blue card) and green Foundation card (or Foundation Funds Database).
  
• Place your completed pledge card in the return envelope and give back to your department representative.
