Case Management

Patient centered case management plays an important role in our clinic, and ultimately involves nearly every member of the CNYOHCC staff.   The case management team consists of a registered nurse and 2 LPNS, who work  to address patient needs and carry out care plans developed for each patient  .

The case managers are primarily responsible for communication with patients between visits, listening to them, conveying information on Workers’ Compensation and other types of insurance. 

Case managers also interface between patients and practitioners, empowering patients who are most able to speak for themselves, and troubleshooting and problem-solving for those who are less able to do so. 

They advocate for patients internally at the clinic and externally in the Workers’ Compensation arena and in other areas, including psychosocial issues, thus providing a vital segment of CNYOHCC’s unique services.

Case management services include, but are not limited to:

  1. obtaining authorization for diagnostic testing and treatment,
  2. coordinating private insurance and Workers’ Compensation benefits for payment of testing and treatment,
  3. communicating with patients' attorneys,
  4. coordinating disability benefits requirements for patients who are temporarily or permanently disabled.   
  5. minimizing delays within the clinic, to overcome barriers in the Workers’ Compensation system, which often results in in real hardship for many of our patients.
