Patient Navigation and Educational Services
Funded by the March of Dimes, our Center has developed and has begun to implement a model for a case management/patient navigation and education service that operates in conjunction with the Syracuse Community Treatment Court (SCTC).
Project staff work in concert with the SCTC employees, as well as the staffs of the various OB providers in the city.
The project is multidimensional
Through referrals from SCTC, we identify women who are at risk for a poor birth outcome due to the negative social, economic, cultural, safety and health factors associated with prostitution and/or substance abuse and encourage them to participate in the program.
Preconception/Interconception Education
The project staff provides information about Preconception/Interconception health including Folic Acid education to all women under the age of 45 who are involved with SCTC.
We provide each participant with a copy of the "Preconception/Interconception Health" worksheet that was developed at the Center. We review the document with each woman, and encourage her to bring it to her women's health appointments.
