Admission Requirements: Professional Doctor of Physical Therapy Program

The admission requirements for the Professional Doctor of Physical Therapy Program are as follows:

  • Baccalaureate Degree from an accredited college or university
  • While we do not have a minimum GPA requirement, preference is given to GPA's of 3.3 or higher.
  • We recommend shadowing to gain an understanding of the career.
  • Completion of Prerequisite Courses

How to Apply 

  • Professional Doctor of Physical Therapy applicants must submit both the Physical Therapist Centralized Application Service (PTCAS) and Upstate Applications
    Applications:

    • Physical Therapist Centralized Application Service (PTCAS) application -
      • Two letters of recommendation
      • Official College Transcripts
    • Once Upstate receives your application from PTCAS, we will e-mail you your Login ID and PIN that will allow you to access our Secondary Application. Please do not create an account on your own from our Admissions Login page as that will not allow you to access the DPT Secondary Application. 
  • Application deadline for PTCAS is January 16th
  • Application deadline for the Upstate Supplemental Application and the $65 application fee is February 1
    • The Upstate Supplemental Application is only available upon completion of the PTCAS Application
  • Personal interview, if invited.
  • Incoming students will be required to submit a background check after being offered admission, and prior to enrollment at SUNY Upstate. All admitted students are admitted conditionally until background check results are obtained and reviewed by SUNY Upstate.

Veteran Applicants

SUNY Upstate, along with all 64 SUNY state-operated campuses, are proud to waive the application fee for US Veterans, Active Duty or Reserve military and their dependents! This waiver will cover the application fee for Upstate applications and SUNY applications (some programs do not require a SUNY application). Proof of military connection must be provided. Acceptable documents include: 

  • DD214 documents
  • Military Identification cards
  • A combination of documents providing proof of military connection and proof of dependence
    • Ex. Spouse's DD214 or ID Card and tax forms indicating you as a dependent. 

Please note, this is a NYS directive – it is for SUNY app and secondaries – it does not apply to AMCAS, CASPA, PTCAS, etc.
