How to Apply:DPT—Physical Therapy Professional Doctoral Program

EARLY ADMISSION APPLICANTS

High school seniors applying for the 3+3 or 4+3 Early Admission DPT program must submit the following items no later than February 1st.

• College of Health Profession DPT Application
• $50 application fee
• Official high school transcript
• Official SAT and/or ACT scores
• Two letters of recommendation from a Guidance Counselor and/or teachers (preferably science). A letter of recommendation from a Physical Therapist is highly recommended.

Applicants MUST apply to their undergraduate institution of choice, simultaneous with their SUNY Upstate application to be accepted to the 3+3 or 4+3 Early Admission Program.
